  • The National Disability Insurance Scheme provides funding to Australians with permanent and significant disability to support independence, wellbeing, and community participation.

  • You may be eligible if you are aged between 7 and 65, live in Australia, and have a permanent and significant disability. Children under seven may be supported through the NDIS Early Childhood approach. Find out your eligibility here.

  • An NDIS plan outlines your goals and the supports you can access to achieve them.

Support is available through Early Childhood Partners, Local Area Coordinators, or Support Coordinators to help you prepare and use your plan.

  • The NDIS Access Request Form is the official application used to request access to the National Disability Insurance Scheme. It collects information the National Disability Insurance Agency (NDIA) needs to decide whether you meet the NDIS access requirements, including age, residency, and disability, or if you need early intervention.

To apply for the NDIS, you can:

Download the Access Request Form from the NDIS website and complete it with supporting information.

Contact the NDIS at 1800-800-110 to request a form and assistance.

Visit a local NDIA office to collect a form and seek help with completion.

The form asks for personal details, proof of residency and citizenship, and information about your disability or need for support. Your treating health professional usually completes a section outlining how your disability impacts your daily life, or you can include recent reports and assessments as supporting evidence.

You can submit the completed and signed form, along with all supporting documents, via email, post, or in person.

After submission, the NDIA will review your application against the access criteria and make an access decision.

  • The NDIS planning process is how you work with the National Disability Insurance Agency (NDIA) to decide your goals and the supports funded in your plan. Once you are accepted as an NDIS participant, the planning process helps identify reasonable and necessary supports that match your personal goals, needs, and circumstances.

Your planner will talk with you about what is important to you, what you want to achieve, and the supports you currently use or may need. This can happen in person, by phone, or via video call, depending on your preference and availability.

    During planning you will discuss:

    Your aspirations and long-term goals
    Daily activities and routines
    Health and wellbeing needs
    Current supports and any gaps
What support do you need to meet your goals

After this conversation, the NDIA will draft a plan that outlines the supports you are eligible for and how funding is allocated across support categories. You have the opportunity to review the proposed plan and provide feedback before it is finalised.

Once your plan is approved, you can use your funding to engage registered providers like BMe Supports to deliver services aligned with your goals.

You can also work with a support coordinator to help implement your plan and connect with the right services and providers.

The planning process is designed to be collaborative and focused on your goals, giving you more control over how your supports are shaped and delivered.
