Specialist Support Coordination
A higher level of coordination for participants with complex circumstances who need specialist assistance addressing significant barriers within their support environment.
Your first appointment
We start by understanding your goals, strengths, current supports and any gaps or barriers. Together, we identify priorities and establish what effective Support Coordination should look like for you.
How we work
We work collaboratively with you and, where you choose, your family and support network. We can assist with provider connections, service coordination, preparation for NDIS conversations and practical problem-solving across your supports.
