Support Coordination

Support Coordination is a capacity-building support funded through your NDIS plan that helps you understand your plan, connect with services, and build the confidence to manage your supports over time. At BMe Supports, we take a person-centred approach that starts with your priorities and focuses on turning funding into practical outcomes.

What Support Coordination helps with

Support Coordination supports you to understand your NDIS plan, connect with registered providers and community supports, build skills to manage services, reduce stress, and respond to challenges that arise across your supports.

Your Support Coordinator works with you to clarify goals, identify suitable providers, and build a team aligned with your values and needs.

How can I determine whether Support Coordination is included in my NDIS plan?

Support Coordination is funded through the Capacity Building Supports budget in your NDIS plan. You have Support Coordination funding if your plan includes one of the following line items.

  1. Support Connection, Level 1
    This is short-term support designed to help you build confidence to link with services and understand how your plan works.

  2. Coordination of Supports, Level 2
    This provides ongoing assistance to coordinate services, manage providers, resolve issues, and strengthen your capacity to manage your supports independently.

  3. Specialist Support Coordination, Level 3
    This is for participants with high or complex needs who face significant barriers such as mental health challenges, trauma history, justice involvement, housing instability, or complex service systems.

First appointment

Your first session focuses on understanding your goals, strengths, current supports, and any gaps or barriers. Together, we agree on how Support Coordination will support you and map a pathway that fits your life rather than a generic checklist.

Levels of Support Coordination

  • Support Connection helps you build confidence to connect with informal, community, and funded supports.

  • Support Coordination supports you to coordinate services, manage service delivery, and increase independence.

  • Specialist Support Coordination is available to people with complex needs who face significant barriers, including those across intersecting health, legal, and justice systems.

How we work

We collaborate with you, your family, and your support network. We assist with connecting to providers, advocacy with services and schools, plan review preparation, and maintaining consistent support delivery. You remain in control while we provide structure, follow-through, and problem-solving.
