A working relationship, not just a list of services

Someone who sees the whole picture.

Sacha combines her experience as a social worker with the practical work of Support Coordination. Her relationship-focused approach brings the person, their family and their wider team into the conversation.

That means starting with what life looks like for you, not simply moving through the pages of your plan. What is already working? Where are the gaps? What would you like to be different?

The aim is to make your options clearer and support your decisions, while helping the people around you work towards the same priorities.

You should not have to become an NDIS expert to ask for help.

Bring your questions, the things that feel confusing and the supports that are not quite working. You can start there.

Whether you are new to your plan or trying to get existing services working better together, ask about taking the next step with Sacha.

What Support Coordination can help with

Turn your plan into practical steps.

Support Coordination helps you understand your funded supports and build confidence in arranging them. The work is agreed around your circumstances, goals and available funding.

Understand your plan

Talk through your funded supports, service agreements and priorities. Work out what to put in place first and what questions still need answers.

Find support that fits

Explore provider and community options with your preferences in mind. Compare what is available and make informed choices about who you work with.

Keep supports connected

Coordinate with your chosen team, address gaps or service difficulties and review what is working as your needs change.

Help to be heard.

With your consent, Support Coordination can help you communicate your priorities to providers, prepare for plan discussions and make sense of the next steps. It does not replace your voice or make funding decisions on your behalf.

Your providers are your choice.

BMe also provides other services. Choosing BMe for Support Coordination does not mean you need to use BMe for those supports. You can keep providers you value and consider options outside BMe.

Any relevant connection or conflict of interest should be explained and managed with you. The decision about which suitable provider to use stays with you.

What happens next?

A clear start, at a manageable pace.

  1. Tell us what you need

Share your preferred contact method, location and the help you are looking for. An initial question does not need to include your full plan or clinical reports.

  2. Check the fit together

Our team checks Sacha's availability, service suitability and your Support Coordination funding, then explains what information is needed.

  3. Agree on the next steps

If you choose to proceed, we confirm the service agreement, any required NDIA request for service and intake arrangements before support begins.

After receiving a referral, our team will contact you within 2–3 business days. A referral does not confirm an appointment, availability or funding approval.

Questions are welcome

Before you get started.

Can I ask to work with Sacha?

Yes. Tell the intake team you are enquiring about Support Coordination with Sacha Murphy. We will check availability and suitability with you. Sending an enquiry or referral does not reserve an appointment.

Is Support Coordination the same as plan management?

No. Support Coordination helps you understand and organise your supports and connect with providers. Plan management focuses on financial administration, such as processing provider invoices and tracking spending. This page is for Support Coordination.

Do I need Support Coordination funding?

Before funded services begin, we need to confirm that your plan includes suitable Support Coordination funding and that the requested work is within scope. You can ask the intake team about the information needed to check this. BMe cannot guarantee a funding decision.

Can Sacha help when my current supports are not working?

Tell us where things are getting stuck. Support Coordination can help you review your current arrangements, discuss concerns with your chosen providers and explore other options. Any work is agreed with you and within the available service scope and funding.

Where is support available?

Sacha's listed area covers Geelong, the Bellarine Peninsula and surrounding communities. Remote support is also available where suitable. Share your suburb or town and state so the team can check delivery options and current availability for your location.

Do I have to use BMe for my other supports?

No. You can use BMe for Support Coordination without choosing BMe's other services. We can discuss your existing providers and suitable alternatives. Relevant connections or conflicts of interest should be made clear so your choices remain informed.
